Do you know someone whose life was changed thanks to a strong relationship with their primary care provider?We're looking for compelling stories that show the power of effective primary care — from the perspective of patients or primary care clinicians (physicians, nurse practitioners, and others).

Selected stories will be shared through short video interviews as part of an upcoming campaign highlighting the importance of sustained investment in primary care.
