Open Notes Proxy Access Collaborative

The first quarterly meeting of Open Notes' new learning collaborative on portal proxy access for older adults will take place on Monday, July 18, 1-2pm EST (10 - 11 am PST). We welcome anyone interested in working together to improve access to healthcare information for care partners of older adults.

Open Notes is convening a learning collaborative for healthcare organizations and staff, patients, clinicians, vendors, and care partners to come together and learn from each other. The learning collaborative will discuss best practices for increasing care partner adoption of the patient portal and other health information technology innovations aimed at helping care partners and patients get the information they need. We’ll talk about healthcare organization-specific strategies and patient and care partner-centered needs.
